A famous films about split personality disorders are "The Three Faces of Eve" (1957), "Fight Club", "Psycho", and the story of "Dr Jekyll and Mr Hyde" . I would think the use of mirrors (a convention of music videos) could work because you need to communicate the notion of the personality disorder.

The film is in black and white, a noir thriller with superb chiaroscuro lighting, and hopefully this could offer you ideas for indoor shots.
The idea of binary opposites is promising. You could contrast claustrophobic indoor shots with the idea of a trackless forest....
